Insurance

We accept most major health insurance plans as well as Medicare.

Please check with your individual plan for the most current information about our status as a provider with that plan.

Please present your insurance card at each appointment. If your insurance has changed, please notify us and bring your new insurance card to your appointment.

Co-payments are due at your appointment time. You can write a check, pay with cash, Visa or MasterCard. Statements also include an area for credit card payments, if needed.

If you have any questions about insurance or billing, please call our business office at 952-893-9109.
